1. Get an eye exam and contact lens fitting.
2. Within 90 days of your exam date purchase the required quantity of lenses from the list above. You will receive your Cibavision rebate form with your contact lenses.

• Offer valid only in the United States and Puerto Rico.
• Offer limited to one (1) fitting rebate per person during the promotional period and further subject to the Terms and Conditions on the rebate form. Purchase must be made within ninety (90) days of eye exam.
